What to Include in a Bill of Lading Template

Whatever template you use, a straight bill of lading usually needs the same core information. Requirements vary by carrier, so check theirs before you ship:

  • Shipper and consignee: names, pickup and delivery addresses, and a contact at the receiving location.

  • Carrier and reference numbers: carrier name, BOL number, PO or customer order numbers, and the PRO number once it is assigned.

  • Handling units and packages: how many pallets, skids, cartons or crates, and the package type.

  • Weight: actual scale weight per commodity, including pallets. Wrong weights are a common reason for re-billing.

  • Freight description, NMFC item and freight class (for LTL shipments), plus a hazmat indicator when applicable.

  • Special instructions: liftgate, do not stack, appointment required.

  • Payment terms and signatures: prepaid or collect, shipper signature, and the driver's signature at pickup.

Shipping to a large retailer? Many require the standardized VICS bill of lading format. Check your customer's routing guide before you print.
